A renal biopsy is performed on a 31-year-old female
who presents with increasing edema, chest pain, and a rash
for the past 6 months. She has an increasing serum creati-
nine level, and proteinuria with red blood cell casts is
found on urinalysis. The light microscopic appearance of
the renal biopsy is seen in the figure. It is stained with
periodic acid-Schiff stain. Which of the following antibod-
ies, if present, is diagnostic of this condition?
(D (A) Scl-70
(3 (B) Anti-Sin
(9 (C) Jo-1
O (D) Anti-HLA-B27
O (E) Anti-centromere

  (B) These are the "wire loop" glomerular capillary
lesions of lupus nephritis. Anti-Smith and anti-double-
stranded DNA are specific for SLE. However, anti-Sin is
present in only 25% of cases. Scl-70 is a marker for dif-
fuse systemic sclerosis. Jo-1 is most specific for polymyo-
sitis. HLA-B27 is seen with ankylosing spondylitis. Anti-
centromere antibody is seen most often with limited
scleroderma.
