In adults with sickle cell anemia, hydroxyurea
increases hemoglobin F production, which inhibits the po-
lymerization of hemoglobin S. Because hemoglobin F lev-
els are high for the first 5 to 6 months of life, patients with
sickle cell anemia do not manifest the disease during this
period. Globin synthesis decreases with the thalassemias.
The hemolysis associated with sickling promotes erythro-
poiesis, but the concentration of hemoglobin S is not
changed. Hydroxyurea does not shift the oxygen dissocia-
tion curve nor change the oxygen affinity of the various
hemoglobins.
