  A 72-year-old retired bank teller comes to the office with a 3-month history of bilateral shoulder and hip weakness. He complains of aching and stiffness, especially in the morning. He notes difficulty in rising from a chair and combing his hair. The onset of these symptoms has been gradual. The patient also describes increasing fatigue, malaise, and a 5-10 pound weight loss over 5 weeks. His temperature is 37.0 (98.6 F), blood pressure 110/80 mm Hg, and pulse is 65/min. Physical examination is unremarkable. Although the patient complains of weakness, his motor strength is intact and there is no muscle tenderness on palpation. Initial laboratory shows a normocytic, normochromic anemia. Additional laboratory studies will most likely show
A. elevated erythrocyte sedimentation rate (ESR)
B. elevated muscle creatine kinase (CK)
C. positive antinuclear antibody titer (ANA)
D. positive cytoplasmic antineutrophil cytoplasmic antibody (c-ANCA)
E. positive rheumatoid factor titer
