  Location: Office
C.C: High blood pressure is noted in a 55 year-old male patient during his routine medical check-up.
Vitals: Pulse: 72/min; B.P.160/95 mm Hg; Temp. 98.50F; R.R: 17/min.
HPI:
A 55-year-old white male comes to the doctor’s office for his routine medical check-up which shows high blood pressure. He denies any headache, chest pain, shortness of breath, palpitations, leg swelling, dizziness, or syncope. His bowel movements are regular and his bladder function is good. He has no other complaints. He has no known allergies. He is not taking any medication. For the last 25 years he has smoked 10 cigarettes per day. He drinks alcohol occasionally and does not use illegal drugs. He is sexually active with his wife. FH: Father died at the age of 50 due to MI. Mother is 85, alive, and healthy. The rest of his ROS are unremarkable.

  this pt has got stage 2 Htn... so I would start him on diuretics....

any comment will be appreciated..

  STAGE 2 HTN USUALLY HAS 2 DRUGS TO BE STARTED WITH HCTZ+ACEi (if no hyperkalemia and serum Cr is normal)

  these steps are done after we diagnose..we cant diagnose on first day on routine checkup

  we can call him back on the following day and if the reading is still the same-- we can go ahead with medicines..
I dnt think that we should wait for 1 month before starting the Rx for stage 2 Htn....

  Anyone knows about specific guidlines for Htn follow up?

It is very high yield topic for Step-3..... they ask very often in CCS too these days...

  PRE NORMAL 2 YEARS

NORMAL 120-139/80/89 ----> 1 YEAR

STAGE I 140-159/90-99 ----->2 MONTHS

STAGE II >160/100 -----> 1 MONTH BUT IF >180/110 ADMIT N TREAT
