  Thank u so much Sarim........And I really enjoyed ur answers........Again Thanks....But NBME 6 Section 1 items .....6( i go for Figrastim .....) and 30( i go for continued cigarette smoking) here is the explanation....

Filgrastim...as it is only used to treat the neutropenia....Sagramostim is also used but not commonly to treat isolated agranulocytosis....It is for both granulocyte and macrophage lineage ..as for the side effect both drugs can cause all the side effects on the qustion stem....

For number 30...Strongest contraindication is continued cigarette smoking ...the others are relative contraindications ...here is the

  I mean (for 6) the side effect is the same both filgrastim and Sargramostim....and the question is not about the side effect...rather the question is about ...Which of the following is used for treating Agranulocytosis.........

  And Sarim.....Section 2 ...item 47.....i go for gonadoblastoma ...which is common in 90 % of cases with other chromosomal abnormalities....such as turner's syndrome (the case in the question )....

check out the page #186 under "Psychosocial criteria" and "substance abuse".

He has mild mental retardation and acting as if he has OCD but doesn't say that he was Dxed with OCD,But it does say that He has a Hx of Major Depression.

I think this is one of those Qs that they through in step-1 that no matter how much u read from review books u will never get it right unless u knew it for sure from other source.

  I think smoking and substance abuse are also included in psychosocial problems .....here is the classification of psychosocial problems


Types of Psychosocial Problems
A. type classifications

1. internalizing disorders – turned inward; emotional and cognitive
symptoms i.e. depression, anxiety and psychosomatic disorders

2. externalizing disorders – turned outward; behavioral problems or
acting out i.e. delinquency, drug and alcohol abuse, truancy

the stem is talking about "First Dose Side effect " of Sargramostim. please read carefully the links that i posted.Also note the "Hypotension'.

block 2 # 47: so u think we are most likely to find Gonadoblastoma rather than "Streaked Ovaries"(Atrophy) ?

  Streaked Ovaries means not Atrophy....It is a form of Aplasia ...which is totally different from Atrophy....
Here is the link for Gonadoblastoma(btw...some books say it is part of dysgerminoma ...which is common in Turner's syndrome)....
