  First of all, thank you sanmarcos for posting your answers you think was right or were unsure on.

I am posting my own answers (I paid for the extended NBME 7 form).
-- There were some that sanmarcos posted as unsure in which either I confirmed as the answer or either marked with a note on the side.

Discussion of answers is more than welcomed! (especially if you think my marked answer is wrong...but some I know for sure is right because I got it right on the NBME 7)

21-25 EGBBA #24 is NOT major depressive disorder because patient does not display suicidal thoughts....this answer should be bipolar, answer B

31-35 AECFC #34 count repeats for normal Q allele - it is 6 repeats so go onto graph, the risk is 0%; count repeats for expansion, it is total of 15 repeats so intersect it with the line and it corresponds with 60% risk
36-40 DCDDD
41-45 BBABB
46-50 CCADC

  q block 4 question 4- my first controversy

WHY IS IT AA
Progesterone will dec the risk of endometrial hyperplasia......the ans shud be DD.....r u sure its AA
On the contrary progesterone will inc the risk of thomboembolism

The markings on X-axis are given in decreasing order and marked -log dose
what does that mean
I was thinking the asnwer shud be potentiator.......please clarify me....thanks

Is it just chronic hypertension......why is the heart so enlarged.....

  block 4 question 24 - i agree its bipolar disorder........maybe type I coz there is no h/o of depressive episode previously.....but now he is acting maniac

i am confused between uncal herniation and cingulate gyrus herniation
the picture in FA shows that epidural hematoma can cause any of the 3 kinds of herniation

Q#43 - r u sure its BB.......the bicarb levels is almost normal(or maybe a little low).......i think it is uncompensated resp acidosis.....i think that was the answer marked by samomcos also

i am not sure whats the sedative drug causing combined resp and metabolic acidosis


Q#45 - is it a subpleural bleb which ruptured to form a pneumothorax......whats ur diagnosis

Q#27 - any idea what those enzymes and substrates are.......i thought it was tyrosine hydroxylase acting as a rate - limiting enzyme in the reactions......correct me if i m wrong

for Q17 in block 4

I think after so much clinical scenario he himself told us that its a Gm -ve organism
So He is experiencing the symptoms of Gram negative shock with is usually due to endotoxin LPS which will stimulate production of TNF alpha from macrophages

But i have no idea what the 4 black round papular lesions on the legs were.
