How do we differentiate sarcidosis from TB ?
one more
Tall T waves becomes inverted after few days can some one explain the reason ?
because in wiki it says Tall T waves and U waves means cardiac ischemia

I think (other than risk factors and clinical presentation of weight loss and night sweat, ..., and acid fast , etc) the main clue in the question stem is caseous granuloma which indicates TB otherwise whenever the question says bilateral hilar lymphadenopathy and non-caseous granuloma it means sarcoidosis.
About T wave, I'm not very sure. But I know, for the sake of USMLE, tall T wave and U means Hyperkalemia. Also tall T wave happens in hyperacute ischemia which changes to T inversion shortly.
