  A full term neonate has repeated episodes of bilious green vomiting during the first day of life and does not pass any meconium. He was the product of a normal pregnancy, to a mother not known to have any major illness. Except for abdominal distention, the infant is normal on physical examination. X-Ray films show multiple air-fluid levels and distended loops of small bowel. A sweat test is normal. Which of the following is the most likely diagnosis?

A. Duodenal Atresia
B. Intestinal atresia
C. Intussusception
D. Meconium Ileus
E. Necrotizing enterocolitis

Duodenal atresia does not have abdominal distention when compared to jejunal or ileal atresia

Due to "Vascular Accident in Utero"----->Ischemia of the respective Bowel segment.

Bowel assume "Spiral Configuration" like an Apple Peel


Duodenal Atresia---------------------------- NO Abdominal Distention
Intestinal Atresia(Jejunal,ileal atresia)---- Abdominal Distension


-This is not the presentation of Inrussusception.

Just thought to share in duodenal atresia u get double bubble sign and in small bowel obstruction
u get multiple air fluid level so B is rt??....Im i correct.. Thank u.

yes it is correct.

Abdominal X-ray:

Double Bubble and NO Distal Air/gas-------------- Duodenal Atresia and Annular Pancreas
Double Bubble and Distal Air/gas ------------------ Malrotation

Multiple Air-Fluid Levels and Distended loops thru out Abd----Intestinal Atresia (non-duodenal)
(No Double Bubble)
