  Once & 4 all.
You need to know what u been given & what been ask of you in status, let's roll ;

The status unknown/none for age 19-64 y/o-> 1st you give PCV13 then you give > PPSV23 (at least 8wks later) - then you give PPSV23 (at least 5yrs after 1st PPSV23).

The status unknown/none for age 65+ y/o -> 1st you give PCV13 then you give > PPSV23 (12 months/or 1yr after PCV13).

The status known PPSV23 for age 65+ y/o -> you give PCV13 (at least 1yr after you PPSV23) then you give > PPSV23 (6-12 months/or 1yr after PCV13 after PPSV23).

The status known PPSV23 for age under 65 y/o -> you give PCV13 (at least 1yr after you PPSV23) then 2nd dose you give PPSV23 (at least 8wks after PCV13 AND at least 5 yrs after 1st dose of PPSV23) -> 3rd dose PPSC23 given at age of 65 (if at least 5 yrs has gone by since last dose of PPSV23.

Now if status PCV13 know -> no addition PCV13 are needed and remember at least 8wks MUST elapse before pat receive a dose of PPSV23.

FIN.

  Very clearly explained, better than UW.. Thank you

This is what I got:

8 wks, 5yrs, @65 PPSV23

>65 yo ----

Known(PPSV23)----1 yr later = PCV13 ---- 6-12months(according to UW) later PPSV23

Unknown--- PCV13--->6-12 months later ---PCV23

Hope I didn't make mistakes. Thank you!

  Ok so UW question says.. .

42 yo F has had all childhood vaccines and a Td after injury at 31; she does not recall any other vaccine since then. She's having acites from chronic alcohol use. Her serology is negative.

ANs is to give her: Hep A & B vaccine, Tdap, influenza, and PPSV23.

so in this pt her Pneumococcal vaccine is unknown. right?

  Sorry for late reply,

Now that you give me case, here the vaccination history foggy but pat painted in the chronic pic:) here. It's a long list that our pat fall into the "HIGH-RISK" category. RT?> on that list all must get PPSV23 & also pat bracket 19-64 y/o.
Remember we give PCV13 to all pats EXCEPT with history of (lung/heart/liver disease/DM/smoker cigarette/alcoholism) our pat fall also in that cat.

If I ask you pat already got a PPSV23, you remember we need to wait for PCV13 bz of immunogenicity that slows down by recent PPSV23. Just for your info.
