  Hello a friend sent me this question and we are confused to what the answer is.

A 70 year old male presents to the clinic with a painless neck mass Physical examination reveals a right-sided submandibular lymphadenopathy. Biopsy is consistent with squamous cell carcinoma. Which of the following is the most likely primary site of the cancer?

Lymph Nodes
Tongue
Parotid Gland
Stomach
Thyroid Gland

I initially thought parotid gland but apparently its not. We are not sure if its lymph nodes or thyroid gland.

So two things first:
1. I know that there is a squamous cell thyroid carcinoma, so it could be from the thyroid gland or
2. The submandibular gland has no intraparenchymal lymph nodes and the tumour involvement in the submadibular gland must be from a locally involved LYMPH NODE or Primary Tumour.

So is it Lymph Nodes or Thyroid Gland????? or is it something else. HELP PLEASE
