  I think it will, one because the USCE will be different for two specialities and you might not have enough experience for the specialities individually, ex: surgery and medicine. Also when you rank programs you are allowed to put less programs for each specialty in your top 10 for rank order. Say if you want a competitive specialty and you have those programs in your top 10. The chances of getting into those programs are low and they decrease your chances of getting into the other speciality.

  But but what if you get like 5 6 interviews only?
I’m concerned that do you appear confused to the program if you apply to 2 different specialties in the same program? Like do they rank you lower thinking that you’re not sure?

  Some departments communicate amongst themselves about candidates. So there is always a chance they’ll notice you applied to both departments. And if you only get 6 interviews rank the programs carefully. Ranking is based on a lot of things, commitment is a big one and how interested you are in a field is another one. And if you do apply to two, make sure you have enough USCE for both. Ex: one IM rotation and one FM rotation.

  There wont be any decrease of chances until the both programs you have applied wont communicate with each other.

  The chances of them communicating are slim, but there is a chance. Especially if there is an ethics committee overseeing all the residents applicants.
