A 26-year-old man with a history of recurrent bouts of cholecystitis is found to have bilirubin-enriched gallstones following cholecystectomy. The patient says that both his father and one of his brothers have had similar trouble with gallstones at an early age. He is anemic with evidence of hemolysis and spherocytes on peripheral blood smear. A direct Coombs' test is negative.
