how can we know that if the breast cancer is malignant?

In tumors <1 cm in diameter, the size is the important prognostic factor;

in tumors between 1 and 2 cm, grade is the most important prognostic factor;

in tumors >2 cm, the status of the lymph nodes is the most important prognostic factor.

Metastasis in breast cancer is primarily a function of size owing to the presence of more tumor cells and a greater chance for dediferentiation as it grows larger.

Other important prognostic factors include the grade and lymph node status. Additional prognostic factors include the status of the estrogen and progesterone receptor assay, the S phase of the tumor, and whether the tumor is diploid or aneuploid.
