if the SP is lying down or sitting for back pain,what for ROM.dowe make him stand?
NO ONE???
another q,low back pain patient with pancreatitits,what physical exam shud we do?abd or MSK??

Abdomen, chest, Heart, and mucus ( Icterus, color, edema)

we will post a response but hope you read it quickly before it disappears

in all cases do heart and lung if time permits do abd

if you have a case with back pain and pancreatitis

yes the patient needs to stand up
but first

inspect, palpate, have them sit up and move to the left then to the right, have them lean forward and lean back (extension/flexion) then they stand and touch they toes. you must do cva tenderness as well and gait

then do a full neuro exam

its take less than a min to check the abd, it is important that you do so if they have pancreatitis
but its unlikely you wil see a cse with a double dd, if its a back pain case then thats what it is

GA: NO ACUTE DISTRESS
VS WNL
HEENT NORMCEPHALIC, ATRAUMATIC, PERRL, NO ICTERUS, NO PALLOR
NECK SUPPLE, NO LAD, THYROID WNL
CARDIO RRR S1 S2 NORMAL
CHEST CTAB
BACK: WNL, NO POINT TENDERNESS, IN SPINAL PROCESS, CERVICAL, THORACIC OR LUMBAR, MILD B/L CVA TENDERNESS, TENDER LUMBAR PARASPINAL MUSCLE B/L
NEURO: L4 L5 S1 MOTOR INTACT 5/5 LOWER EXT B/L
L4, L5, S1, LIGHT SENSATION TO DULL AND SHARP INTACT B/L
ACHILLES, PATELLAR REFLEX 2/4 B/L
PULSE: 2/4 DP,PT,B/L

usually you dont want to make the sp "do" anything

if they say they cant stand because it hurts too much then note that in your findings in the pn

not able to check for rom due topain or restricted rom upon standing
if they say they cant stand, gently try to convince them and if they refuse be gentl and say I understand the pain is difficult for you and I will do everything I can to help you today.

do not be forceful at all with the sps

treat them like little babies Smile

what is ROM?
ROM means Range of Motion. Part of muscleskeletal exam.
