where is the passport booklet no. written?...is it the passport file no.???

Specific travel plans:
If you booked a flight already, just go ahead and write all information, including ur flight number, date of arrival, arrival city, departure city, departure date: write approx last week of June 2011. Thts when ur visa will expire if given for one year. departure flight: u may leave blank.
Stay in U.S: write any hotel, any relative or friend's house. As a last resort u may write name of ur hospital with its address.
Previous visa number: Its the red colored number, written on right lower quadrant of ur visa.
Ten printed:
If u went for visa couple of years back, I would suggest write NO. If u appeared for visa, last year, write YES.
Not employed: write that u prepared for application, exams, interviews and now will start residency from the given date.
