A 56 y/o brought to the ER cz of 2 wks hx of progressive shortness of breath and leg swelling.He now require two pillows for sleeping cz of shortness of breath when lying flat.He has had 10 lb wt gain during this period.He has 15 year hx of type 2 diabetes and HTN that are poorly controlled despite the treatment with variety of medications.Two months ago he began receiving new diabetic medication that has been titrated upward during the past month.Crackles are heard at the lung base bilaterally.Cardiac examination shows a S3 gallop.There is a 2+ peripheral edema of lower extremities.
