72 yo M w/ H/o HTN n Hypercholesterolemia. Meds ramipril, statin, aspirin. V/S normal. Examination harsh right carotid bruit. Carotid DUS 30% stenosis on the right and 10% stenosis on left. Next step?
a- continue current regimen
b-add clopidgorel
c- carotid angiography
d-MRA
e-Carotid endarterectomy


Why not prevent future complicaiton by adding clopidgorel? Why A?
First of ask q: what are indications for clopidogrel?
This pt doesn't have Stroke or MI(angioplasty).
Answer is A.
Management of stroke: if pt don't take antiplatelt -aspirin, If on asprin add dipiridamol or switch to clopidogrel alone.
There is no indication for C D E

b-add clopidgorel ------- used in CAD and stroke
c- carotid angiography ------ don't need this bc pt already had DUS
d-MRA ------ this is done for Aorta related conditions
e-Carotid endarterectomy --- needs >70% stenosis to do this
