we all know that vomiting leads to hypocholeremic hypokalemic metabolic alkalosis....
however my question if a patient is on PPIs and later on sometime develops intractable vomiting ,what would be the effect on acid base level in blood??/

On PPI stomach pH still remains acidic (4-5.1) so pt will still lose acid but in much smaller amounts. How will that affect patinets pH? I guess it will depend on the half life of the PPI and on the time extent of vomiting.
If short half life and pt with intractable vomiting, ie he cant take the next PPI dose the stomach will return to its regular acidity.
Im not 100% sure on what im saying though

I assume so. Since the stomach is still acid pt will be losing acid, however it will take longer time for alkalosis to occur.
However I dont think one will ask in exam this question. Because everybody is different and may be variation of PPI half life and effect.
