if cystic fibrosis transmembrane regulator on chromosome 17 is defective then u have a problems with secretions they become thicker and blocks the ducts in the glands that were making fluids in the pancreas ..the exocrine part...so if u block the lumen there is a back pressure on the pancreas and it atrophies resulting in malabsorption....
its just like atrophy of cortex and medulla in kidney when there is a back pressure related to urine..the same thing happens here in pancreas when there is a back pressure due to blocked ducts..by thick secretions ...>atrophy of pancreas and other glands too....
