Its in rural place and so those who have families or like cities will face problems. While currently the waiver job is for 3 yrs.......there are talks to extend it to upto 5 yrs!

well the waiver can be 3-5 years depending on how u apply for greencard. there r 2 ways you can apply for gc . one is thru national interest waiver which takes 5 years and you can file this urself and ther other is employer specific ( ur employer file it on ur behalf)which takes three years but lawyers advice you to stays 5-6 mths more after getting gc. so it can also take 3.5- 4yr. Waiver jobs are availabe after residency if you know where to look.

Like sherwood said they are in rural areas and are very competitive to get into...ie large number of doctors with the J1 visa that just finished residency are vying for a very limited number of visa waiver positions...and since this is the case not all can get the waiver job...meaning that those who dont get the job have to go back to their country. Its pretty risky under the current rules, imagine how it would be if they changed the rules (since it is 3 years away) and put a bigger cap/limit on the number of available positions...to me its kind of scary...but I guess it depends on the individual.
