This is dilated cardiomyopathy with congestive failure most probably- DOC furosemide- but may be digoxin could be too- any further opinions?

the first step in CHF is preload reduction w/ Diuretics, Oxygen if that doesn't work you give dobutamine as a intropic positive, digoxin takes too much time to work, you can used as a outpatient and ACE inh later.
