Metro we cannot use in first trimester.............so paromomycin is the best choice throughout pregnancy..........but we can use Metro during pregnancy other than first trimester

MRI v/s Bone Scan...............Very good question.........equal sensitivity............MRI allows better differentiation of overlying soft tissue infection and bone.................otherwise which ever is available faster...................also if pt has CI to MRI like pacemaker etc your obvious choice is MRI

Bone scans, however, may be difficult or impossible to interpret in patients with contiguous-focus osteomyelitis because adjacent inflammation, infection, or postoperative changes may result in false-positive results. written in mksap(can u explain this)?

MRI is the choice..
limited role of bone scan is to identify occult infection and multifocality. You cover entire body.

When u know, that there is osteomyelitis, do MRI to evaluate extent of disease, soft tissue involvement, abcess, joint involvement.

But if u suspect a foreign body like nail induced OM, CT scan is complementary to MRI, not substitute.

My advice, forget about bone scan in infection.
