You are called to see a patient in the intensive care unit who was admitted with pneumonia and intubated for hypoxemia and acidosis. You learn that he is a 57-year-old man with severe asthma for which he was being treated with chronic steroids along with multiple inhalers. He has no other medical problems. Currently his temperature is 39 C (102 F), blood pressure is 75/40 mm Hg, pulse is 140/min, and respirations are 17/min. His physical examination is significant for bilateral wheezes with decreased breath sound at the right base. Despite aggressive hydration with normal saline, his blood pressure remains low.
