1.do we need to do mmse in headache case?
2.sp says pain is 8/10-how we show empathy?'
3.RLQ pain whivh worsen with movement-in FA they write 'hypoactive BS'-certainly we'll not find that in exam
4.MVA pt with bruise in chest-should we percuss
5.if a pt with severe abd pain doesn't want to sit how can we exam heart and lung

1- depending upon the positive that that you get, if the guy is old , if tumor or temporal lobe encephalitis seems to be one of the causes.
2-depends on how the patient is looking, are those the episodes that he described when he grades them as 8/10, and currently he is not in such a pain or at the point of asking, he has this degree of pain, one can ask is there anything that you can do to help apart of medication which you can give once you are done assessing..(actions speak louder than words)
3-depends on pain , one has to percuss as dull note on percussion aids in diagnosing hemo thorax, so tell them you will be as gentle as possible.
4- provided you have time , do it lying down but first let the drape be brought down after completing abdominal exam, never put patient in discomfort to do a examination.
