Anyone knows the reason why you must screen for adrenal insufficiency in a patient with a central hypothyroidism?.
UWORLD says that you must but I don't understand the reasoning.

coz of all hormone def ---adrenal def is worst..pt may die without coticosteroid bt nt wid hypoth,
hypo gnrh....

to add to above...........if you give levothyroxine to pt and due to adrenal insufficiency if the patient is not having steroids....then pt wont be able to cope up with the stress which which will occur with increased metabolism that will occur due to levothyroxine...

The hypothalamus and anterior pituitary act as central in regulation TRH-->TSH for thyroid and Adreniocorticotropic relaeasing hormaone --> adrenocorticothropic stimulating hormones, and direct effects on the target organ the adrenals and thyroid glands.

Patients with central hypothyroidism must also be evaluated for secondary adrenal insufficiency since administration of thyroid hormone increases the catabolism of cortisol and may precipitate adrenal crisis.
