in a test probably (B).
in a real life situation my first question to him will be (A) in order to understand what the patient is thinking and to test his knowledge about smoking; then i will tell him the long term health consequences of smoking.
but still tricky question.
this is my humble opinion.

The answer is C... 5 %
The inference shows the 95 % CI and there is significant difference between the means (p < 0.01). Therefore, I can say with 95 % confidence that the actual mean is in the specified X and Y interval. There is only 5 % doubt that the results might have occurred merely due to a chance.

because I put B as the answer choice on the exam and go it wrong, the only other possibility would be D.

Okay, so option E doesn't seem to make any sense! ruled out.

Option B and C are more of yes/no questions.

Option A and D are open ended ones.

Here the doc has already started talking about smoking cessation.

So, option A makes more sense.
