COWS is for coma pts. TO CHECK IF THE CORTEX IS INTACT or not.

COLD --mimics a lesion

WARM --mimcs a stimulus.

cold water-- slow phase moves the eyes to the same side ohhhhh if cortex is intact it works and suddenly the eyes move to the opposite side which we call it as nystagmus.

warm water-- slow phase moves the eyes to opposite side and if cortex intact ,suddenly the eyes move to the same side which is again called as nystagmus...

COMA means brain stem totally gone.

Pseudo Coma means mid brain is still working pt.can communicate with his eyes as third nerve is in mid brain..

When a pt. closes the eyes while standing with feet together, the visual and the cerebellar components of propioception are removed, so you are testing the dorsal columns. Swaying with eyes closed is a positive romberg'e sign indicating a lesion in the dorsal columns. The cold water caloric test mimics a brainstem lesion by inhibiting the normal reflex response. ( COWS: Cold water : Nystagmus toward the lesion with quick phase to opposite side, Warm water: Nystagmus to opposite side with quick phase to same side).
