if there is classical case of limb ischemia / acute arterial occlusion, any criteria to decide whether we should go for Heparin therapy in next step or an emergency surgery ??

you can go for TPA and heparin but best inital thing would be to do CT angio or MRA to delinate the site of occlusion and also start heparin. Consult vascular surgery---they will decide to go for Thrombolysis with TPA or Embolectomy.
