Though the Jason one was helpful, I think in this question symptom is more of like kidney stone. so the answer could be b. this q is vague.

b for archer hematuria, is clear that the pt has kidney stones

I think it is B for the reason that Patient presents with flank pain which couldnt be a lower UT even though there is no cast in the urine. An evaluation for upper UT is CT scan while an evaluation for lower UT is cystoscopy or urogram. This patient has kidney stone.
