cluster;100% o2, sumatreptan,lithium ,prednison
tension;nsaids, rest

MIGRAINE---mild--head ache withour nausea and vomitting---nsaids
moderate--head ache lasting for 6-24 hours with nauses vomitting and the functionin is impaierd---nasids/ergot/sumatripton plus an emetic

severe--throbing headache 24-48 hours with vertigo,vomitting--patient is grossly incapitacted during the atack---ergot/sumatripatan plus anti emetic

for prophylaxis--proponal
amitryptiline or beta blockers
flunarizine or calcium blosckers
methysergide

if patient vomits the drug----cafergot--ergotamine tartarate img +caffenine 100mg
given rectally as suppositories or dihydroergotamine mesylate subcut or im can help

cluster headache---100 percent oxygen
if not available sumatripton/lithium/prednisone

tension headache---nsaids---behavioral relaxation

*Tx:Acute episode of Cluster HA: 100% O2 is the best Tx.
Sumatriptan or Ergot : for acute Cluster or Migraine
Tension HA: NSAIDs, Relaxation---------refractory: muscle relaxant
Note : Sumatriptan constrict the vessels--C/I in pt w CAD
*Prophylaxis :
1-Migraine: Avoid trigger.
-(when > 3 attacks/month): B-Blocker (propanolol, timolol), Valpric acid & Methysergide = 1st line
Remember SEs of Methysergide? ------
Valvular & retroperitoneal fibrosis
2-Cluster HA : Prednisone, Lithium, Ergotamine, Methysergide & Verapamil
