Uncooperative patient - zana

The patient is uncoopretive. you couldn't do the physical exams.What phrase do you put on P/E ? Just write uncooperative on GA ? What should we write for example on Heart examinition and if we can mention that exam on work up or not?

Most of the time Sp's allow you to examine.When they say she or he cant allow you to do the examination you have to thell them I am here to help them and It is important to do this examination to find out the exact cause for his or her illness.Most of the time they will allow you to do the examination.
After all this stuff if they dont allow you to examine you can mentioned patient did not allow to do the examination of inspite of the councelling or request.

I mean if the patient doesn't allow you to exam for example her heart , How you mention it on P/E notes? for example like this:
GA : ..................
HEENT : ...................
CHEST : ...............
HEART : The patient doesn't allow the exam
................
and then do you mention it on work up if you think it is nessarry ? or not?

Your way is correct.If it is relavent you have to mention it as you stated
