The scenerio that BDJ mentioned happened to one of my friends too, but the point is it can happen even when u go for a visa few months after starting ur residency. If your name has been shortlisted for Security check they will always do it irrespective of whether u are starting or have started residency.
I would also not agree with world is flat, as they don't have to give you a specific reason for visa denial. If for some reason he doesn't like ur face he can still reject you.

There is always some risk once you leave United States without a valid Visa stamped on your passport...wherever you go, there is a possibility of getting the Visa rejected!!!!! And if you have anything in common with some terrorist or someone "wanted", that will trigger the alarm and you will not be granted the Visa until the security check is clear which may take about 3-6 months or more !!!!

Another option for you is that if you have another valid Visa now - B2 or F1 - you can go to Canada or India before you start your residency, and try to get your H1 stamped....in that case even if you are rejected H1, you can always return to US within the validity period of F1 or B2......and start your residency on H1 and get it conerted at some other time...may be 1 or 2 years after residency....

Again this is like beating the dead horse.... all these kinds of phobia dont help much. What if they place you on "no fly list"? What if your H1B is not approved because of security concerns
? What if they want to do a random security check when go to the embassy after 1-2 years of residency? Drop the PGY-3!!!? or dont go to your home country till you get your green card (or to be 1000% sure till your Citizenship-8/10 years)... Technically if you overstay or extend B1 visa, they may question your intention of returning to your home country on a J1 (not H1B) visa. Security checks/risks are not in your hands. Just go and get your H1B stamped and hope that works out. If you are not sure then wait till you get your green card to see ur family!!!!!(6-7 years)
