This patient has acute dengue fever acquired from a mosquito bite while he was in the Caribbean. The 1-week incubation and symptoms including severe myalgias, petechiae, and epistaxis are typical. Diagnosis is best made by IgM ELISA or paired serology. The disease may progress into a case of hemorrhagic fever with severe shock and gastrointestinal hemorrhage. The differential would include rickettsial illness transmitted by a tick. Acute HIV seroconversion can present with fever, adenopathy, and myalgias; however, the HIV PCR would be postitive with symptoms (although the HIV antibody test would be negative). Exposure to rodent urine could cause leptospirosis.
