A 75-year-old patient with advanced metastatic
prostate cancer and a long history of renal failure has severe
bone pain. He is given meperidine. Two days later
he develops a generalized seizure. What is the likely
mechanism of this complication?
(A) Buildup of meperidine

Answer is B

(Meperidine is as such CNS depressent but its metabolite nor meperidine is CNS stimulant. Person with normal renal fn ..large oses required to have tonic clonic seizure..but as in this case patientt with renal failure very small doses can ppt seizure)

-- Meperidine in high doses for long time can also causes renal failure (esp when administered thorugh traditionally route) --> so that can lead to seizure. But here pt took drug for only couple of days , so collection of metabolite NORMEPERIDINE is cause of seizure.
