how is mediastinitis different from pstpericardiotomy syndrome?is it important?how will u differentiate the two?

_ Postpericardiotomy syndrome is the episode of pleuritic chest pain, dyspnea and fever after cardiac surgery.

came across this in uw.post pericardiotomy syn is autoimmune whereas mediastinitis is infectious.one is treated with steroids n another with surgical drainage,debridement n abs.but clinical symptoms has been described almost same.so wanted to know if anybody knows how to differentiate it
