cardiac troponins as they are markers of myocardial necrosis.also, CPK MB is fairly reliable. these donot follow the pattern as in acute MI, as the myocardial necrosis in myocardial contusion is immediate, not like the ischemic damage, where troponins are elevated in 6 hours.if the initial troponins are not elevated, it is less likely to be myocardial contusion

blunt trauma is the most common cause of contusion, like a sternal fracture.but echo is not very reliable, as it doesnt distinguish between intramural hematoma and contusion.hypokinesia is just a marker of myocardial dysfunction, not a very good marker of myocardial contusion.

Diagnosis: ECG may show ST elevation, or may be normal. Initial cardiac enzymes may be normal. If no other injuries requiring hospitalization are present, the decision to hospitalize the chest-injury patient to rule out myocardial contusion must be based on the clinical picture ” for example, the presence of substernal pain or a bent steering column.
