U cant say that its either the mucus plug or Ca.
But in case of plug u can remove it and can visualize the bronchial tree and in case of Ca u can do the biopsy. So in either case ur approach is better than CT which cant facilitate u as broncho.

These are lines which i saved from UW step 2 CK !
If a patient's pneumonia fails to resolve at the end of two weeks antibiotic therapy, a bronchoscopy is indicated. Bronchoscopy would help obtain cultures, visualize the bronchus, and do a drainage.
A CT scan will help if a mass or lung abscess is suspected. A CT scan will also help to determine if an empyema has set in.
