I don't know what answer you need, but to me it looks like a regular doc that describes the whole story of your exams with scores including step3 and now CS.

Hi, I would like to ask how to request and pay for USMLE transcript in OASIS ? I requested it in MyEras but then don't know what to do next. Forgive me if my question sounds stupid. Thanks alot.

I wasn't able to take my CS during my eligibility period because of some health problems. Send explanation with dr report to ECFMG and they gave me a full credit back so I was assigned to a new eligibility period when I took and passed CS, which means I don't have any attempt.
Yesterday I checked the status of my Step 3 application, and saw my exams history. For CS I have 2 rows - one is that I had eligibility period but didn't schedule exam, and the second is that I passed during my new eligibility period. So I was curious how this is gonna look on the paper which goes to PDs...

queendr, that's enough to request transcript in MyEras. They are gonna do the rest, and you will be able to pay when you apply to the programs.
