is it ok to mention in a pediatric LOR that docotor is recomending me to a "IM programme in your hospital" because i am applying only for IM but i have a US LOR in padiatrics.

yes it is ok, but if it up to you tel your LOR writer not to advice you to any specialty, because you may need it to apply to multiple specialties.
