Do we have to do visual acuity exam during eye exam with pocket chart (Snellen chart) or we can write in the work-up. and if yes, is pocket chart is provided at the center.

do it with ur fingers and mention that on the Pt. Note - but also mention Visual Acuity testing using Snelles in the INvestigation section on the PN ...
