Pulm thromboembolism will be very sudden to cause these changes.

The lungs adapt to COPD by undergoing hypoxic vasoconstriction, which eventually results in Pulmonary Hypertension. This will lead to Hypertrophy and Dilation of the Right Ventricle and Right atrium, i.e. Cor Pulmonale.
