A 32-year-old G4P2 woman at 33 weeks' gestation presents with contractions every 4 minutes, which have persisted for 6 hours despite hydration and resting in the left lateral recumbent position. She denies vaginal bleeding, leakage of fluid, or decreased fetal movements. She has been treated in the past month with intravenous fluid replacement and morphine on presentation to triage. At 30 weeks' gestation she was hospitalized for observation, magnesium sulfate tocolysis, and betamethasone administration; at that time her cervix was 2 cm dilated, 75% effaced, and at 0 station. Today on examination her cervix is 3 cm dilated, 75% effaced, and at 0 station. She reports having chills earlier this morning and says she has not felt well for a couple of days. Her temperature is 37.3°C (99.1°F), and her other vital signs are within normal pregnancy values. She denies dysuria, cough, chest pain, diarrhea, leg pain or swelling, or sick contacts.
