Seems like delirium tremens to me
Usu. occurs After 2-5 days after last drink( usu seen in alcoholic, since he is hospitalized for cholecytectomy,i am assuming he missed his drinks)
Hints: anxious,tremors,confused,restless
So give benzodiazepam
R/o ceftriaxone since no s/s of sepsis,infection (low grade fever)
R/o buspirone Why would u give buspurine for confused pts
R/o naltrexone (doesnt seem opioid intoxication--other hints like pinpoint pupils,cns depressants so would be sedated and not restless....)
R/o pentolamine (except for bp 160/100,other symptoms are not consistent with pheo)
And no use giving sertraline...
So i would go with alcohol withdrawal delirium tremens and give diazepam!!
Correct me if i am wrong!!
