Surgery Form 3 - Help Needed! - ssg725

1) 8 hrs after undergoing TURP, 62 yo M has a Hgb conc of 7.5 g/dL. Transfusion of heterologous packed RBCs is begun. Two hrs later - chills and fever. He has recieved 200 mL of transfusion. Temp is 39 deg C (102.2 deg F), pulse is 120/min, RR is 18/min, and BP is 120/70. Which is the most likely explanation for these findings?

a) ABO incompatibility
b) Bacterial overgrowth in transfused blood
c) IV catheter infection
d) Preformed antibodies to leukocyte antigens
e) Rh incompatibility

My thoughts were originally transfusion reaction, so A or E, but since they would both cause a transfusion reaction, I had to rule them out. My next thought was B or C, I guessed B and it was wrong. D I think is wrong bc they are packed RBCs, so leukocyte antigens wouldn't matter.

Dd even that it is packer traces of leucocytes are there. you need to wash them to remove all leucocytes or irradiate
