Not a significant problem if you can explain them completely and reasonably, and if they are not extremely long.
Or a big red X in your application if that affects your YOG cutoff or is not explained by work or research.

The main concern that programs have is that such applicant with a gap would have been away from academic medicine for too long and may not be as manageable, used to the routine or compliant as a fresh graduate.
