I am in "deep trouble..."

I am in "deep trouble..."
 I am ("thought that I was...") a good student, historically speaking.
IMG- pakistani. But despite my topper status in medical school, here in these exams, i am unable to
move the needle past an nbme 201. Kindly consult with me if you have concrete solutions that will correlate to my specific case.
Thank you usmleforum members for your bountiful guidance that may lead me to
assured success in these refractory, "impossible to figure out-" examination patterns.

This exam is not like our Exams in Med School (8hour MCQ). The test requires a level of understanding complex mechanisms and the combination of memorization. Send me your skype name and I could ask you what your study plan looks like.
