i failed my cs
Inshallah when i pass it will attempt be mentioned on ECFMG certificate

Yes they are mentioned and also you have to mention them in the CAF

Standard ECFMG Certificate
ECFMG issues the Standard ECFMG Certificate to applicants who meet all of the examination and medical education credential requirements. Applicants must also pay any outstanding charges on their ECFMG financial accounts before their certificates are issued. Standard ECFMG Certificates are sent approximately two weeks after all of these requirements have been met. Currently, ECFMG sends the Standard ECFMG Certificate to the applicant™s address of record by Federal Express. If your address of record with ECFMG is a U.S. post office box, the certificate will be sent by certified mail.

The Standard ECFMG Certificate includes:

the name of the applicant;
the applicant™s USMLE/ECFMG Identification Number;
the dates that the examination requirements were met;
the date that the certificate was issued; and
if applicable, the dates through which the passing performances on certain exams remain valid for the purpose of entry into graduate medical education. (See Validity of Examinations for Entry into Graduate Medical Education).
Applicants may use the Standard ECFMG Certificate to enter an accredited program of graduate medical education in the United States.

International Medical Graduates (IMGs) applying to programs in California may submit the Applicant Evaluation Status Letter issued by the Medical Board of California to ECFMG for scanning as one of the four letters of recommendation. Call it "California Letter", and assign it to the California programs.

Please do not send any additional documents, such as a copy of your ECFMG certificate, curriculum vitae, personal statements, copies of your USMLE Step scores or a TOEFL score. Do not send copies of your medical diploma, or your license or certificate of registration from your home country, as the ECFMG Status Report also indicates the status of your credentials. Please, do not send copies of personal documents, such as your U.S. Alien Card, Birth Certificate or Name Change Certificate. These documents will not be scanned or transmitted.

USMLE scores can only be reported as part of an official USMLE transcript. Therefore, ECFMG can no longer include USMLE scores in the ECFMG Status Report. If you took USMLE Step 1 and/or Step 2 for ECFMG Certification, the ECFMG Status Report will only show month and year that you passed these exams.

The only scores listed on the ECFMG Status Report are those for the ECFMG one-day examination, FMGEMS, VQE, PART exams, or FLEX ” if you received your ECFMG Certification based on any of these older medical science exams.

If you passed Step 1 and Step 2 after being certified by ECFMG and you want the programs to know your USMLE scores, you need to request a USMLE Transcript. Go to MyERAS, Documents and click on USMLE transcript.

The Status Report will not contain any failing scores or other non-passing attempts.

The USMLE transcript contains your entire USMLE history on Step 1, Step 2 CK, Step 2 CS, and if taken Step 3. It includes all attempts: passed, failed incomplete, etc. It is not transmitted automatically. You need to request its transmission, pay $50 and assign it to your programs.

Fifth Pathway applicants may want to list their Fifth Pathway Certificate as one of their letters. Submit a sharp copy of the Fifth Pathway Certificate to ECFMG

I don't think it will appear in the cetificate. But it will show up in your transcript.

it doesn't appear in your certificate but it will be mentioned in your transcript that will be sent to the pograms by the other hand, program directors will see it.
