The questions with 79yo woman, chest pain & agitation for 4 hrs, systolic ejection murmur at the apex, ECG shows ST-elevation in anterolateral leads, CXR shows enlarged cardiac silhouette.
Either acute coronary syndrome or acute pericarditis.
