as far as I know, you can not find the attending like job after the fellowship.
I think you must finished Step 3 to apply the fellowship position.
however, even after that, you still will not have the license you need to practise clinical in US.
however, if you take 5 years fellowship or other clinical experiences (what i know is for orthopaedic surgeon), and you have finished a resident program in China, and you have a title of associated professor, then you can join the board for specialist.
but, I donot know what will happen after that.

I think it is better for you to take one year fellow then match again.

Congratulations to all the friends matched .
Best wishes to all that did not get through it this year. I've heard of many stories, like with low scores 82(2nd attempt)/87/pass/not take, 3 interviews, but matched this year. Please don't, never give up. Everyone knows, we people came from China is very smart and work hard.
Mine:90/88/pass/not take, 4 interviews, matched a relatively competitive specialty (University based program, but not with a good reputation). GC holder. 7 years clinical experience in China, 5.5 years research experience in US.I applied very late, got ECFMG certificate on the last day of Nov. I think the main reason I got matched is I prepared well for the interviews. And my spoken English is good (though my vocabulary is very low).
