Pat with history of depression present w 6 to 7 gouty attacks every yr at present. You been treating pat w febuxostat. You find that your pat highly allergic to probenecid. Which of the following would the best for yr pat ?

a)Allopurinol
b)Sulfinpyrazone
c)NSAIDs at low doses
d)Pegloticase
e)NSAIDs at high doses

For the Q here, I would pick "E" .....NSAIDS at higher doses have anti-inflammatory effect as well.

cardio: I thought you are asking me what to give for treatment of the gouty attacks he was having, meaning management of the gouty arthritis at that time, which is NSAIDs.

When the inflammation is controlled, then yes, allopurinol as Dr Goljan says.
If that answer is not correct, then I really don't know what could be the answer Smile

Can please look at pg 645 under *pegloticase is a procine recomb uricase metabolize urate -> H2O soluble allantoin is best for "refractory" pat.
BTW im the only member can claim pegloticase Smile
